On Vita-Salute San Raffaele University's 2026/27 lists for the International MD Program, the 86th and last EU place of the first round scored 41.00 out of 60, and by the notice of 15 October 2026 enrolment had reached the 183rd candidate, at 35.00. The non-EU list's 64th and last first-round place scored 48.75; its last notice IvyTest recorded, on 6 October 2026, reached the 102nd, at 46.50, and non-EU enrolment closed on 21 October 2026.

In short

  • EU: the first round's 86th place scored 41.00 out of 60, and offers have since gone down to the 183rd candidate on the Definitive Ranking List, at 35.00 (15 October 2026).
  • Non-EU: the 64th place scored 48.75, and the last notice IvyTest recorded reached the 102nd, at 46.50, before enrolment closed on 21 October.
  • The EU list went so far down because most of its top candidates never asked for a place: only 360 of the 598 ranked declared they wanted one, and only 36 of the first 86, so the 86th place went to the 182nd on the original ranking and the 183rd to the 334th.
  • So the non-EU line sat about 11 points higher than the EU one, the opposite of what the places alone would suggest.
  • Chemistry is where marks were lost: 203 of the 360 EU candidates on the definitive list got 2 or fewer of their chemistry questions right, which brings an extra learning requirement (OFA).

Everything here comes from San Raffaele's own documents: the EU ranking list of 598 candidates, the EU Definitive Ranking List of the 360 who then declared they wanted a place, the non-EU final list of 391, both OFA lists and the enrolment notices, read on 21 October 2026 or on the dates given. No candidate's code is reproduced.

This covers the six-year Medicine and Surgery programme taught in English in Milan (International MD Program, IMDP), with its separate EU and non-EU calls. It does not cover the Italian-taught Medicina e Chirurgia, which has its own test and ranking.

San Raffaele cut-off scores 2026: where each list stopped

San Raffaele runs two separate calls, each with its own test sessions, ranking and places: 86 for EU citizens and 64 for non-EU citizens. Both rank on a candidate's best score across the sessions they sat, out of 60, with no minimum. Each list is read at two points: its last place in the first round, and the furthest position an enrolment notice has opened since, as places were turned down.

Scores out of 60: +1 for a correct answer, −0.25 for a wrong one, 0 for a blank. The EU positions are on the Definitive Ranking List, not the first ranking. A notice opens a place to a position; it does not say the candidate took it.

From 598 to 360: the EU declaration of intention

The EU call adds a step the non-EU one does not have. After the ranking list of 598 candidates came out on 27 March 2026, every candidate had to declare between 9 and 22 June whether they wanted a place; the 360 who did make up the Definitive Ranking List of 26 June, and only they can be offered one. The other 238 dropped out, including all eight candidates at the top of the ranking and 50 of its first 86.

So positions on the two EU lists are not the same thing. The table shows where each definitive position stood on the original ranking: the 86th place, the last of the first round, had been 182nd, and the 183rd, reached in October, had been 334th.

The three lists, position by position

Plotted against position, the non-EU list sits above both EU lists from the first places down: its 64th scored 48.75, while the EU ranking's 64th scored 47.25 and the Definitive Ranking List's 43.00. The Definitive Ranking List falls fastest, because its strongest candidates had already left the full ranking when they did not declare.

The notices, 2026/27

Once the first-round places were taken or turned down, San Raffaele opened the next positions by notices on its rankings pages, each giving a few days to enrol online. The page shows only the latest notice, so these are the ones IvyTest read on the day: they are not every notice of the year. On the EU side, positions 1 to 86 could enrol from 1 to 20 July 2026; on the non-EU side, positions 1 to 64 confirmed with a €1,500 advance by 9 April 2026, and the list moved on between 10 April and 30 June before the autumn notices.

How the 2026 scores were spread

The two full lists, before any declaration, compare like for like: every candidate who sat at least one session. The non-EU field scored higher across the board, with a median of 40.75 against 36.25, and 134 non-EU candidates reached 45 against 107 on the larger EU list. Both had about 40 candidates at 50 or more.

Science and the additional learning requirement (OFA)

San Raffaele's OFA lists give, for each candidate, the number of correct answers in Biology, Chemistry, Mathematics and Physics, each out of at most 6, and flag an OFA, an extra learning requirement, for any subject with 2 or fewer right. Chemistry stands out on both lists. On the EU Definitive Ranking List 203 of 360 candidates were flagged in Chemistry, with an average of 2.33 correct; on the non-EU list, 128 of 391, with 3.23.

SubjectEU: flagged (of 360)EU: average correctNon-EU: flagged (of 391)Non-EU: average correct
Biology384.16544.01
Chemistry2032.331283.23
Mathematics783.83454.52
Physics1153.15733.78

The EU OFA list covers the 360 on the Definitive Ranking List; the non-EU one covers all 391 ranked, so the two are not the same group of candidates. No call states how the 24 science questions divide; the lists count at most 6 correct per subject.

Ties at the boundary

Scores move in quarter points, and ties sit right on the lines. The EU 86th place shared 41.00 with the 87th; the EU notices of October stopped inside a tie of eight candidates at 35.00, positions 178 to 185. On the non-EU list eleven candidates shared 48.75, positions 63 to 73, across the 64th place, and seven shared 46.50, positions 100 to 106.

The call orders equal scores by the logic and problem-solving questions first, then reading comprehension, then Biology, Chemistry, Mathematics and Physics, and finally the younger candidate. A headline score alone does not say which side of the line a candidate fell on.

Is 35.00 the San Raffaele cut-off?

Not as a final figure. On the EU side it is the score at the furthest position a notice has opened, on 15 October 2026, with enrolment open until 20 October, and the list may still move. On the non-EU side, 46.50 is the furthest position of the last notice IvyTest recorded before enrolment closed on 21 October. San Raffaele does not publish the last score actually admitted.

What the lists do settle is the shape of 2026/27: a first-round EU place took 41.00 on the Definitive Ranking List, and a non-EU place 48.75, and candidates well below both still received offers as places were turned down.

What this means for a 2027 applicant

San Raffaele had not published its 2027/28 calls on 21 October 2026. In 2026/27 the test was 60 questions in 80 minutes, entirely in English: 36 on logic, problem solving and reading comprehension and 24 on Biology, Chemistry, Mathematics and Physics, scored +1, −0.25 and 0, with the best score of up to two sessions counting. If the format holds, sitting both sessions is worth it, and so is the science block: Chemistry is where the field lost most.

For EU candidates the declaration of intention matters as much as the score: the list that offers places is made only of those who declared, so a score that looks short on the ranking list can be well placed on the definitive one.

Questions

What is the San Raffaele IMDP cut-off score?

In 2026/27 the first round's last EU place scored 41.00 out of 60 and the last non-EU place 48.75. As places were turned down, offers reached 35.00 on the EU Definitive Ranking List (183rd, notice of 15 October 2026) and 46.50 on the non-EU list (102nd, notice of 6 October).

Is there a minimum score for San Raffaele medicine?

No. Places go down each ranking by score, with no pass mark. Separately, a candidate with 2 or fewer correct answers in Biology, Chemistry, Mathematics or Physics is admitted with an extra learning requirement (OFA) in that subject.

Why is the non-EU cut-off higher than the EU one?

Because the EU list lost most of its top candidates at the declaration of intention: only 360 of 598 declared, and 36 of the first 86. The non-EU field also scored higher overall (median 40.75 against 36.25).

What is the declaration of intention?

An EU-only step between the ranking list and the places: every ranked candidate must declare, in June, whether they want a place. Those who do form the Definitive Ranking List, and places go down that list.

How many people apply to San Raffaele medicine in English?

The 2026/27 lists rank 598 EU and 391 non-EU candidates, 989 in all, for 86 and 64 places.

Does San Raffaele publish who was admitted?

No. The lists show anonymous codes and scores, and the notices say which positions may enrol, not who did.

Method

Every list was read in full from the PDFs on San Raffaele's rankings pages: each row's position and score, checked to run from the first to the last position without a gap and in descending order. The EU Definitive Ranking List was matched to the EU ranking list by the anonymous code both print, to find each definitive position's place on the original ranking; the OFA lists were read for each candidate's correct answers per science and OFA flags. Notices were read on the rankings pages on the dates given.

The page shows counts, positions, bands and notices only; candidate codes are not reproduced. The EU Definitive Ranking List and the file named 'Graduatoria definitiva' on the page carry the same 360 rows.

The data

The aggregates behind every table and chart on this page: the three 2026/27 lists as score counts, the definitive list's positions on the original ranking, both OFA lists by subject, and the notices recorded. No candidate-level data.
